ALP CBAT Vision Certificate (Annexure VI-A): the format, how to get it made, and the mistakes that stop you at the gate
No original vision certificate in the Annexure VI-A format, no entry to the CBAT: that is the RRB's notice of 8 October 2026 for CEN 01/2025. Here is the format in Hindi and English, what every box means, who may sign it, and a step-by-step guide to getting it made without a mistake.
RRB notice of 8 October 2026, CEN 01/2025 (ALP): candidates must produce the vision certificate in original, in the prescribed format (Annexure VI-A), at the time of entry for the Computer Based Aptitude Test. A candidate without it is not allowed to attend the exam.
The psycho test has a gate before the screen. On the day of the CBAT, before you reach the computer, you hand over a vision certificate: a one-page form, Annexure VI-A of CEN 01/2025, filled and signed by an eye specialist, with your photograph on it. The RRB's notice of 8 October 2026 says in plain words that without this certificate, in original and in this format, you are not allowed into the exam. Months of preparation for the five tests can end at the gate over a missing seal.
This article gives the format in Hindi and English, exactly as the RRB published it, explains every box, says who may sign it and what the seal must show, and walks through getting it made, step by step, so that nothing is left for the gate to find. The five tests themselves are in the complete guide to the RRB ALP psycho test; the day of the exam is in exam day in the ALP psycho test.
Download the format (Hindi and English)
Print the page you will use on A4 paper, full size, with nothing cut off at the edges. The notice carries the format in both languages; the English one (page 4) is the Annexure as it stands in the CEN, so print that one unless your eye specialist prefers to fill the Hindi one (page 3). Print two copies, so that a slip of the pen does not cost you a second visit.
What the RRB notice says, point by point
The tentative schedule of the CBAT was published on the official websites of the RRBs on 1 October 2026.
Candidates must produce the vision certificate in original, in the prescribed format (Annexure VI-A of CEN 01/2025), at the time of entry for the CBAT. A candidate who does not is not allowed to attend the exam.
Some words overlapped in the format printed in the CEN, so the RRB has reproduced the same format with clarity, and instructs candidates once again to produce this format for the CBAT.
There is no change in the prescribed format. It is the Annexure VI-A originally published in CEN 01/2025 (ALP).
The seal of the eye specialist must clearly indicate their ophthalmology-related education and their position in that hospital as eye specialist.
Point 5 is the one that catches candidates. A signature and a round stamp with a name on it are not enough. The seal has to say what the doctor studied in ophthalmology and what post they hold at that hospital.
The format: Annexure VI-A
The English page, as the RRB published it:
The Hindi page of the same notice:
What each part of the certificate means
The heading. "Proforma for medical certificate to be obtained from an eye specialist by candidates applying for the post of Assistant Loco Pilot against CEN No. 01/2025." This heading is how the gate knows it is the right form. A certificate on a hospital's own letterhead, however detailed, is not this format.
Your name. "I have examined Shri / Smt / Kum ......" The eye specialist writes your name exactly as it is in your online application.
The photo box. A colour passport-size photograph, 3.5 cm × 4.5 cm, the same photograph you used for the online application, pasted in the box and attested by the eye specialist. Under it, your own signature.
The table, four columns. For each, a required standard is printed and the specialist writes the actual observation or value for your eyes:
Distant vision: required 6/6, 6/6 without glasses, with the fogging test, which must not accept +2D. In the fogging test the specialist puts a +2D lens before the eye; a normal eye blurs with it, while an eye with hidden long-sightedness still reads the chart. "Must not accept +2D" means your eye must not read 6/6 through that lens.
Near vision: required Sn. 0.6, 0.6 without glasses. Sn. 0.6 is the reading line on the Snellen near-vision chart, each eye on its own, with no glasses.
Colour vision: required Normal. Usually the Ishihara colour plates.
Binocular vision, field of vision and night vision: required Normal.
The verdict line. "Shri / Smt / Kum ...... fully conforms / does not conform to the above vision standards." The specialist strikes out one of the two. A line with neither struck out is an unfilled certificate.
The surgery line. "It is also certified that he/she has not undergone LASIK surgery or any surgery to correct refractive error." It stays as printed and the specialist's signature covers it.
The specialist's details. Name of the eye specialist, registration number of the eye specialist, place, date, and the signature and seal. Every one of these is a box to be filled; a blank registration number is the commonest gap.
Who may sign it: the eye specialist, the seal and the registration number
An eye specialist, which means an ophthalmologist: a doctor with a postgraduate qualification in ophthalmology (MS or MD in Ophthalmology, DNB in Ophthalmology, or a Diploma in Ophthalmology) working as an eye specialist at a hospital.
The seal must show that qualification and that position. A seal that reads, for example, "Dr. (name), MS (Ophthalmology), Eye Specialist / Senior Consultant, (hospital name)" meets point 5 of the notice. A seal with only a name, or a hospital's general stamp, does not.
The registration number of the specialist, with the state medical council or the National Medical Commission, goes in its box on the certificate.
Signature and seal both, on the certificate, and the specialist's attestation on the photograph.
Not enough: an optometrist, an optician or a spectacle shop; a general physician or any doctor whose seal shows no ophthalmology qualification; a certificate signed by the hospital's administration instead of the examining specialist.
Government or private? The notice names no kind of hospital; it names the doctor. The surest places are the eye department of a government or district hospital, a medical college hospital, or a railway hospital, because their specialists' seals carry the designation as a matter of routine. A private eye hospital's ophthalmologist is acceptable when the seal carries the qualification and the position. Pindel Sir's rule for his batches: read the seal before you leave the hospital, with the notice's point 5 in your hand.
Step by step: getting the certificate made
Print the format. Two copies of the page you will use, on A4, full size, from the Download Now button above.
Collect what you will carry. Two colour passport photographs, 3.5 cm × 4.5 cm, identical to the one in your online application; a printout of your application or registration details, so that the name is spelt the same; a photo identity card; and a printout of the RRB notice, to show the specialist what the seal must say.
Choose the hospital and the specialist. The eye department of a government or district hospital, a medical college, a railway hospital, or a private eye hospital whose ophthalmologist's seal shows qualification and position. Tell them at the counter that you need the Railway ALP vision certificate in the Annexure VI-A format.
Give the examination without glasses or contact lenses. The standard is unaided vision. The specialist reads your distant vision in each eye, does the fogging test with the +2D lens, reads your near vision in each eye, tests colour vision with the plates, and checks binocular vision, field of vision and night vision.
See every "actual observation" box filled. All four columns, nothing blank, in the specialist's hand.
See the verdict line struck. Either "fully conforms" or "does not conform" crossed out, leaving one.
Leave the surgery line as printed. Do not strike it, do not let anyone strike it.
Name, registration number, place, date, signature, seal. All six, and the seal with the qualification and the position on it.
The photograph. Pasted in the box, attested by the specialist with a signature or stamp that runs partly onto the photograph, and your signature under it.
Before you leave the hospital, read the whole page against the list below. Then take two photocopies and a photo on your phone, put the original in a plastic sleeve, and keep it where the admit card will be.
When to get it made
The schedule came on 1 October 2026 and the notice on the certificate on 8 October 2026, so the window is now. The notice gives no validity period for the certificate, so a certificate made today serves for the CBAT; do not wait for the admit card to be released before you go to the hospital. When your CBAT call letter arrives, read it for anything more it asks you to bring, and carry that too.
Mistakes that get candidates turned away or into trouble
A photocopy, a scan on the phone, or a laminated copy instead of the original.
A certificate in a different format: an older print with the overlapping words, a hospital letterhead, a general fitness certificate, or a certificate from an earlier stage of the recruitment.
A seal that shows the doctor's name but not the ophthalmology qualification and the position at the hospital.
A certificate from an optometrist, an optician or a general physician.
A blank "actual observation" box in any of the four columns.
The verdict line with neither "fully conforms" nor "does not conform" struck out.
No registration number, no date, or no place.
No photograph, a photograph different from the online application, or a photograph not attested by the specialist.
Your signature missing under the photograph.
Your name spelt differently from the application.
The surgery line struck out or altered.
If your eyes do not meet the standard
The standard on the certificate is the medical standard of the Assistant Loco Pilot post, and the railway tests the same eyes again at its own medical examination after selection. A certificate that says "does not conform" cannot be mended by another doctor's pen, and a candidate who has had LASIK or any surgery to correct refractive error cannot be certified on this form. If you wear glasses for distance, have your unaided vision read before you worry; many candidates who wear glasses for comfort still read 6/6 without them. Talk to the eye specialist plainly. It is better to know now than at the gate.

▸Is the vision certificate compulsory for the ALP CBAT?
Yes. The RRB notice of 8 October 2026 for CEN 01/2025 says the vision certificate must be produced in original, in the prescribed format (Annexure VI-A), at the time of entry for the CBAT, failing which the candidate is not allowed to attend the exam.
▸Which format is the right one?
Annexure VI-A of CEN 01/2025. The RRB reproduced it with clarity in its notice of 8 October 2026 because some words overlapped in the earlier print, and it states there is no change in the format. The notice carries it in English and in Hindi.
▸Who can sign the vision certificate?
An eye specialist (ophthalmologist). The notice requires that the specialist's seal clearly shows their ophthalmology-related education and their position in that hospital as eye specialist. The certificate also has a box for the specialist's registration number, and it must carry their signature and seal.
▸Does it have to be a government hospital?
The notice names no type of hospital; it names the doctor. What is checked is the eye specialist's qualification and position on the seal, the registration number, the signature and the format. A government hospital or a medical college's eye department is the surest choice; a private eye hospital's ophthalmologist is acceptable when the seal meets the notice's requirement.
▸Can I wear glasses or contact lenses for the test?
No. The standard printed on the certificate is 6/6 in each eye without glasses for distant vision, with a fogging test, and Sn. 0.6 in each eye without glasses for near vision. The specialist records what your eyes read unaided.
▸What if I have had LASIK or any surgery for my eyesight?
The certificate carries the line that the candidate has not undergone LASIK surgery or any surgery to correct refractive error, and the specialist signs it. A candidate who has had such surgery cannot be given this certificate.
▸Will a photocopy or a photo on my phone work at the gate?
No. The notice says the certificate must be produced in original. Keep photocopies for your own record, and carry the original.
▸Which photograph goes on the certificate?
A colour passport-size photograph of 3.5 cm × 4.5 cm, the same one you used when registering the online application, pasted in the box on the certificate and attested by the eye specialist. You sign under it.
▸When should I get it made?
Now. The CBAT schedule was published on 1 October 2026 and the notice on the certificate on 8 October 2026; the notice gives no validity period. Get it made at once, keep the original safe, and read your CBAT call letter for anything more it asks for.
Er. Mahendra Pindel (Pindel Sir), founder and director of Kautilya Classes, has coached RRB ALP psycho test batches for ten years in Jaipur and Jodhpur. This is his method, and how the portal puts it on a screen like the hall.
What RDSO tells candidates to do before and during the aptitude test session, and the ten rules Pindel Sir gives every Kautilya Classes batch in the last class before the CBAT.
RDSO publishes a 15-question FAQ on the aptitude test, in English and Hindi. Pindel Sir goes through the answers that matter for the ALP CBAT: who takes the test, how it is scored, the rules, the mock test in the hall, and the five points RDSO itself asks you to keep in mind.
